In West Kent, a series of hubs are being planned and you can hear more and have your say on what services could be delivered from a local hub at one of three planned events.

 

 

 

 

Staff, patients, the public and stakeholders are all invited to attend one of three public events in June and July to discuss the development of local care hubs as part of our wider local care model.

Some services need to be provided across a larger area for more people, and to be based together, in a hub.  A care hub can significantly expand the support available to people in the community offering health services nearer to home rather than travelling to a hospital for treatment.

Events will be hosted by NHS West Kent Clinical Commissioning Groups (CCG) and their partners on:

  • Tuesday 19 June – Tonbridge Leisure Centre – 6 – 9pm
  • Friday 22 June – Oakwood House, Maidstone – 10 am – 1pm
  • Tuesday 3 July – Stag Theatre, Sevenoaks – 1.30 – 4.30pm

Last year we shared our vision for developing a stronger, better model of care locally working with partners in healthcare, social care, mental health, and the district and borough councils; these events are an opportunity to hear about and discuss the work being undertaken to develop potential solutions for local care hubs in west Kent.  We are looking at which services could be based in a hub and, where they could be based.

We will be discussing the principles and issues we need to consider to ensure the plans are robust and will work for our communities. It is important that the views of patients, public, staff and stakeholders help us shape the model.
